Resource Kits
To help enhance care for persons with or at risk for breast cancer, the HANYS BCDP Advisory Board authorized the development of several resource kits related to specific outreach and care management strategies. Each kit focuses on a particular program developed by or tested at a HANYS BCDP site and provides a comprehensive template to help you implement that program in your hospital.
Each kit contains materials in a variety of Microsoft formats that you can download and customize for your use. In addition, HANYS can help you customize and print bulk copies of certain sample flyers and brochures. An order form is included where this feature is available. Employee Mammography Inreach Program
An employee mammography inreach program can identify undetected cancers among the women in your workforce. The program also offers potential for enhancing employee satisfaction and improving interpersonal relationships.
Patient Navigator Program
Patient navigation programs help breast cancer patients move through the health care system and access appropriate social and financial services. Dr. Harold Freeman, Chairman of the HANYS BCDP™ Advisory Board, initiated the first breast cancer patient navigation program in 1990 at Harlem Hospital Center in New York City.Provider Tools for Bridging the Gap
This kit includes resources and strategies to help health care providers and newly diagnosed patients enhance communication, and establish collaborative partnerships to support shared decision-making and informed choices.MAP: Mammography Advantage Pathway Resource Kit
This kit is designed to help hospitals identify opportunities to increase operational efficiencies in mammography services and optimize financial resources. It derives from a pilot study of four HANYS BCDP hospital sites.
