Palliative Care Series: Integrating Palliative Care into the Nursing Facility Setting

HANYS, with the generous support from the Milbank Foundation for Rehabilitation, is holding a Webinar series for health care providers seeking to initiate or expand access to palliative care for patients, residents, and families.

This webinar will feature palliative care experts in geriatrics, palliative care across the continuum of care, and hospice, who will describe the benefits of, and planning for, the integration of palliative care in nursing facilities. The key elements of establishing or strengthening palliative care service partnerships and integration models in the nursing facility setting will be explored, as well as how to apply palliative care principles to the care of residents and their families.

This webinar will cover:

  • the impact of the current clinical environment and how palliative care is changing to meet long-term care demands in 2015;
  • current palliative care models and their related quality outcomes;
  • barriers to implementing palliative care in long-term care and ways to overcome those barriers; and
  • strategies for building partnerships to build palliative care models.

About Our Speakers:

Kathryn M. Lanz, D.N.P., A.N.P., G.N.P.-B.C., A.C.H.P.N.®

Dr. Kathryn (Katy) Lanz is the Director of Geriatric Services for the University of Pittsburgh Medical Center (UPMC) Palliative and Supportive Institute. She also serves as Director for the Centers for Medicare and Medicaid Services (CMS) Reduce Avoidable Hospitalizations using Evidence-based Interventions for Nursing (RAVEN) Facilities in Western Pennsylvania Initiative, which supports gero-palliative integration into nursing homes across western Pennsylvania. Dr. Lanz has been practicing in hospice and palliative care in various settings for nearly 15 years and received her Doctor of Nursing Practice degree from Vanderbilt University. She is a board certified advance practice nurse in adult, geriatric, and palliative care. Dr. Lanz is the recipient of the 2013 National Board for Certification of Hospice and Palliative Nurses’ Advance Practice Nurse of the Year Award and the 2014 Project on Death in America award that recognizes a nurse who has emerged as a leader in palliative care, and whose work has improved nursing care for dying patients and their families. She specializes in practical approaches to implementation of palliative and geriatric care in community based and long term care settings.

Melanie J. Marien, M.S., R.P.A-C.

Melanie Marien is Director of Clinical Relations and Business Development at The Center for Hospice and Palliative Care, one of the nation’s largest hospice and palliative care programs. A registered physician assistant, her background includes development and leadership of palliative care services across the continuum, including community-based palliative care services. She has a combined 16 years of clinical experience in inpatient and outpatient care settings, including internal medicine, trauma and emergency medicine, neurosurgery, and hospice and palliative care.
