Palliative Care Series - Palliative Care Access in the Community: Home Care and Assisted Living Facilities

HANYS, with the generous support from the Milbank Foundation for Rehabilitation , is holding this Webinar series for health care providers seeking to initiate or expand access to palliative care for patients, residents, and families.

This webinar will explore the key elements of establishing and strengthening palliative care service partnerships and integration models in home care and assisted living settings, and how to apply palliative care principles to the care of residents and their families. This webinar will include:

  • the importance of extending palliative care into the community;
  • models of community palliative care delivery in the home care setting;
  • the importance of palliative care and hospice services in assisted living facilities (ALFs); and
  • evidence-based outcomes of community-based palliative care.

Important Note: This combined home care and ALF program is replacing programs previously scheduled for January 15 and February 19. Registrations for the previous programs are not transferrable. You must re-register online for this February 19 program.

About Our Speaker

Melanie J. Marien, M.S., R.P.A-C.

Melanie Marien is Director of Clinical Relations and Business Development at the Center for Hospice and Palliative Care, one of the nation’s largest hospice and palliative care programs. A registered physician assistant, her background includes development and leadership of palliative care services across the continuum, including community-based palliative care services. She has 16 years of clinical experience in inpatient, outpatient, and community care settings, including internal medicine, trauma and emergency medicine, neurosurgery, and hospice and palliative care.

The Importance of Palliative Care: Supporting the Triple Aim

Hospitals, nursing homes, home care agencies, and ALFs across the state are in the midst of rapid transformation as they seek to achieve the “triple aim” of improving health, enhancing quality, and lowering costs. Research has demonstrated that palliative care helps support the triple aim goals by reducing unnecessary hospitalizations and emergency department visits, improving quality of life, and reducing the cost of care for seriously ill individuals.

Palliative care projects are promoted in New York’s Delivery System Reform Incentive Payment (DSRIP) program because palliative care is a valuable service that results in higher quality, well-planned interdisciplinary treatment that decreases the costs of care and unnecessary re-hospitalizations. It also links diverse hospital departments and services for effective and efficient use of hospital resources.

The New York State Palliative Care Access Act (PCAA) requires hospitals, nursing homes, home care agencies, special needs assisted living residences, and enhanced assisted living residences to provide access to information and counseling regarding options for palliative care appropriate to patients with advanced life-limiting conditions and illnesses. These providers and residences must facilitate access to appropriate palliative care consultation and services, including associated pain management, consistent with patient or resident needs and preferences.
