Population Health Improvement Program (PHIP)

HANYS will host this webinar with representatives from three Population Health Improvement Program (PHIP) contractors. Participants will learn about their activities in different regions, including focus areas they have in common, those that differ based on regional needs, and how PHIP activities will link to the Prevention Agenda and DSRIP.

Last year, the New York State Department of Health selected 11 regional PHIP contractors to promote the Triple Aim of better care, better population health, and lower healthcare costs. Each PHIP will help support and advance ongoing activities related to the New York State Prevention Agenda 2013-2017.

PHIP contractors will convene stakeholders and establish neutral forums to support strategic planning for identifying, sharing, and helping implement best practices and local strategies to promote population health and reduce healthcare disparities in their respective regions.

Three PHIPs will be featured in the webinar:

  • Trilby de Jung, Chief Executive Officer, Finger Lakes Health Systems Agency
  • Lottie Jameson, Vice President, Regional Health Planning and Development, Adirondack Health Institute
  • Janine Logan, Senior Director of Communications and Population Health, Nassau-Suffolk Hospital Council

This webinar is designed to provide value to all members, including those who are not in these three PHIP regions. Within the framework of statewide population health goals, the webinar will explore various regional approaches that can inform activities in other regions. Each presenter will share details about their PHIP activities and how they link to the state’s Delivery System Reform Incentive Payment (DSRIP) program and the Prevention Agenda.
