Preventing Hospitalizations: Dissecting the Top 10 Diagnoses for Nursing Home Residents

National data show ten diagnoses are the repeated reasons nursing home residents are admitted back to a hospital. Join us for this free webinar that will offer strategies to successfully decrease resident readmissions.

Nursing home leaders’ efforts to reduce unnecessary re-hospitalizations improves their organizations’ quality of care performance and assists related initiatives such as the Medicare Quality Assurance and Performance Improvement program, New York State Delivery System Reform Incentive Payment, and contracting with managed care plans.

DETAILS:

Do you know the story behind your facility’s re-hospitalization rate? Many organizations have not done the data collection, review, and analysis of their re-hospitalizations to understand their re-hospitalization performance.

This webconference will present the data, analysis, and tools to help leaders define and understand their re-hospitalization rate.

Albert Riddle, M.D., will present the provider strategies that have successfully helped many leaders successfully address and decrease those rates. This program is a practical application of the concepts learned in July with Denise Boudreau-Scott, and embodies many of the Department of Health (DOH) guidelines for nursing home medical directors and attending physicians and used in the Education, Training, and Technical Assistance (ETTA) Project.

After attending this program, participants will:

  • know the top diagnoses responsible for unnecessary re-hospitalizations of nursing home residents;
  • identify tools and develop strategies for decreasing re-hospitalization rates in nursing facilities; and
  • understand how the DOH guidelines for nursing home medical directors and attending physicians interface with the strategies of decreasing re-hospitalizations.


Dr. Riddle is the President and Chief Executive Officer of the Riddle Medical Group in Haworth, New Jersey. The Riddle Medical Group provides medical direction and medical services to long-term care facilities. Dr. Riddle is a member of the American Medical Director’s Association, American Medical Association, and New York Medical Director’s Association, and has served on numerous advisory boards. He is a graduate of the State University of New York, Downstate Medical Center, and New York University.

This webconference is free to all nursing homes in New York State and funded by ETTA.
