Improving Communication During Patient Transfers

Hospitals across the country are working to improve care transitions. This webinar will focus on communication relating to patient transfers from emergency departments to another facility. Results from an eight-state pilot focused on emergency department transfer communication will be shared, along with information on how measurement and tools related to this issue are being supported by the New York State Department of Health’s Office of Rural Health.
