Improving Hospital Mortality, Readmissions, and Patient Satisfaction through Palliative Care

HANYS, with the generous support from the Milbank Foundation for Rehabilitation, will continue its 2016 Palliative Care Education Series in this webinar featuring nationally recognized expert David E. Weissman, M.D..

This program will address key hospital quality improvement areas: inpatient mortality, readmissions, and patient satisfaction.

Dr. Weissman will provide an overview of the publication, Improving Generalist Care for Hospitalized Seriously Ill Patients: A Resource Guide for Hospital Administrators and Other Staff Charged with Reducing Inpatient Mortality, Readmission Reduction, and Improving Patient Satisfaction, which was published in November 2015.

The 40-page guidebook focuses on the education and systems changes necessary to embed “Goal of Care Conversations” into routine care for seriously ill hospitalized patients. These conversations explore the patient’s wishes and goals for treatment framed within the therapeutic options that are appropriate for the patient’s clinical condition. The book contains a step-by-step guide of key process stages along with a sample electronic medical record template, quality improvement guides, clinician communication guide, and clinician education resources.
