• HANYS and DataGen are partnering to present a webinar that will focus on ways post-acute care leaders can guide their organizations in an evolving health care environment.

• DataGen, renowned nationally for its expertise in healthcare analytics, will outline for HANYS’ members the elements to consider in planning for and demonstrating value as a business partner to entities you are developing relationships with as part of federal and state health reform initiatives.

Program objectives include:

• Understand the payer and consumer perspectives of care pathways and how post-acute care fits within the continuum.
• Discuss how post-acute care provides value in a patient-centered care model.
• Learn about the incentives to reduce post-acute care utilization in new payment models such as bundles and accountable care organizations.
• Discussing options to prove value and retain volume.

This program will address issues such as working with upstream and downstream care partners, proving value in the payment equation, contracting for risk, strategies for achieving savings, and how to build successful partnerships to re-engineer care.
