Open Doors Program—Transitioning Individuals Back to Communities

HANYS is hosting this webinar on the Open Doors Program, part of New York State’s initiative to assist people living in nursing homes and intermediate care facilities. The program offers resources for providers to use in planning discharges of individuals back to the community.
Presenters from the New York Association on Independent Living (NYAIL) will describe how to refer nursing home residents to the Open Doors Program and the resources it provides to help transition and support individuals with intellectual and developmental disabilities to live more independently.
