ACA Requirements for Hospital/Health Home Coordination

The Affordable Care Act (ACA) requires hospitals to coordinate with Medicaid Health Homes to refer patients who are potentially eligible for Health Home services, and to notify Health Homes when a Health Home enrollee receives care in an emergency department or hospital inpatient setting. To help hospitals understand and implement the ACA requirements, HANYS and Greater New York Hospital Association will host a briefing on July 25, from 10:30 a.m. to 12:30 p.m.
Department of Health staff will review the ACA requirements and provide an update on the Health Home program. The briefing will also include a panel of hospital, Health Home and Performing Provider System representatives to discuss current practices in coordinating care for Health Home enrollees.
