This webinar series is complementary and open to all New York State healthcare providers.
HANYS, with generous support from the Milbank Foundation for Rehabilitation, continues our series of webinars for healthcare providers seeking to initiate or expand access to palliative care for patients, residents, and families. The program faculty are nationally recognized experts in the field.
This program will address best practices for integrating palliative care services into the primary care and/or PCMH settings. This webinar will cover the evidence and business case for this approach, strategies for patient acceptance, and examples of successful implementation.
