Join us for this webinar to learn about the Advanced Primary Care (APC) model and how it can help your primary care practices achieve meaningful, patient-centered, change, and can help your Performing Provider System achieve Delivery System Reform Incentive Payment (DSRIP) deliverables.
The APC model of care focuses on achieving the triple aim of improving the patient experience of care, enhancing the health of populations, and reducing the per capita cost of healthcare.
