Home Care: Introduction to HHQI’s Acute Care Rehospitalization BPIP

HANYS will give a brief presentation on the Home Health Quality Improvement (HHQI) campaign, with an overview of their first BPIP on reducing acute care rehospitalizatins. As the state’s HHQI Local Area Network of Excellence (LANE), HANYS is excited to announce that the first BPIP is also now available on the HHQI Web site.
If you are not currently registered in HHQI, please register your agency first, by using the link in the top right corner of HHQI’s Web page. This will allow you access to all the campaign has to offer. Agencies already registered in HHQI can log in to the Web site and click on the link in the top left of the page, “BPIP Download.” Registered agencies can download BPIP in its entirety or can download the section(s) of your choice. Additional resources included with this BPIP include a Hospital Risk Assessment Tool, Emergency Care Plan, promotional posters, and agency success stories.
