Transitions of Care: Effective Information Sharing

Hospitals and nursing homes are responsible for exchanging accurate and timely information in order to facilitate the appropriate and safe transitions of individuals between these settings of care. HANYS is partnering with NYAHSA and NYSHFA on a 90 minute teleconference, about this important and critical step in transitioning individuals safely and expeditiously.

Who should attend

Hospital and nursing home staff with interests in patient/resident transitions and information sharing should consider attending this important teleconference.

What you will learn

Teleconference topics will include:

  • information related to the Health Insurance Portability and Accountability Act and the Health Information Technology for Economic and Clinical Health Act;
  • patient-specific information critical to making Medicare and Medicaid eligibility determinations, and;
  • the shared responsibility and requirements for providers to conduct Preadmission Screening and Annual Resident Reviews (PASARR).

Faculty

Presenters will include association and DOH hospital and long term care program staff.

Continuing Education Credits

Nursing home administrator continuing education credits (CEUs) have been requested from the DOH for this program. The $25.00 fee allows only one (1) person to obtain the credits.

Registration/Cancellation Details

The registration deadline for this program is March 11. Refunds will be issued for cancellations up to three (3) days prior to the program, less $5.00 to cover administrative costs. Cancellations of less than three days or “no shows” will forfeit the registration fee. Cancellations must be received in writing via e-mail at jstewart@hanys.org.
