The Difficult to Serve Patient: An Inter-agency, Interdisciplinary Approach

THIS PROGRAM IS FOR HANYS' MEMBERS ONLY.

HANYS will present a Webconference featuring an inter-agency, interdisciplinary team of health professionals describing the unique initiative that these agencies have developed to create and execute viable, albeit complex, discharge plans for clients with significant social and medical challenges. Representatives from Albany County Mental Health, St. Peters Health Care, and Albany County Social Services will describe how their agencies come together to address and coordinate the myriad barriers this unique population faces in achieving a sustainable discharge back to the community from a hospital stay.
