Care Transitions & Reducing Readmissions

HANYS is pleased to announce New Models of Care Delivery: From Strategy to Execution, a new Webconference series that addresses executive level strategic issuesrelated tomaking the business case for new models of care across the continuum, and integrating collaboration into all systems and operations.

In this era of national and state health care reform, providers are challenged to analyze their organizations’ processes for caring for populations with chronic diseases, and to develop and implement new delivery models of care, payment structures, and quality initiatives. This Webconference series will provide information, ideas, and practices that promote “outside the box” thinking for all levels within an organization.

This final Webconference of the series will explain why smooth transitions of care are essential for preventing hospital readmissions, and will make the case for why a multi-faceted approach is needed to improve transitions. We will discuss an evidence-based, self-management model, the “Care Transitions Intervention,” adopted by more than 500 leading organizations nationwide, and will offer links to valuable resources.

We will also summarize best practices and key lessons from hospitals that achieved very low readmission rates, highlighting the importance of the internal hospital environment, relationships with community providers, and specific care transition strategies, as well as case studies and a synthesis report supported by The Commonwealth Fund, available on www.whynotthebest.organd www.cmwf.org.

Each Webconference will include a question-and-answer session.
