County Health Rankings & Roadmaps: Data for State Health Planning, features Julie A. Willems Van Dijk, R.N., Ph.D., Deputy Director of the Robert Wood Johnson-funded County Health Roadmaps project at the University of Wisconsin Population Health Institute. She will review the County Health Rankings, an annual check-up of a community’s health that serves as a starting point for discussions about population health. The County Health Roadmaps provide community leaders tools and guidance to take action to improve the health of communities. This Webconference will provide an in-depth exploration of how hospitals can use the Rankings and Roadmaps.
