Palliative Care Series - The Patient and Family Discussion, 10 Steps for What to Say and Do

HANYS, with the generous support from the Milbank Foundation for Rehabilitation, is holding this Webinar series for health care providers seeking to initiate or expand access to palliative care for patients, residents, and families.

In this Webinar, participants will learn about the key elements for facilitating these important discussions with patients and their families, with the goals to keep them informed about conditions and what to expect, provide an opportunity to express feelings and concerns, discuss options and expectations, and learn about or revisit preferences for care.
