Palliative Care Series - Advance Care Planning

HANYS, with the generous support from the Milbank Foundation for Rehabilitation, is holding this Webinar series for health care providers seeking to initiate or expand access to palliative care for patients, residents, and families.

Advance care planning discussions are typically reserved for people who are terminally ill or whose death is imminent, yet research indicates that people with serious and chronic illnesses can benefit greatly from these important discussions. This Webinar will review different aspects of advance care planning tools and directives, including the use of best practices for deploying Medical Orders for Life-Sustaining Treatment (MOLST) health care proxies, and other mechanisms that are used to represent and guide the preferences for medical care by patients, residents, and clients.
