HANYS Community Health and DOH: Local Health Department (CHA-CHIPs) and Hospital Plans (CSPs)

This Webconference is for HANYS members, DOH employees, and county health department employees only
HANYS and DOH will host this Webconference covering key findings of DOH's reviews of Community Health Assessments-Community Health Improvement Plans (CHA-CHIPs) and Community Service Plans (CSPs). The plans will be described, examples of best practices illustrated, and next steps outlined.
Webinar participants will:
- understand the state's community health priorities;
- beable to identify at least three characteristics of strong examples of CHA-CHIPs and CSPs; and
- understand how to access technical assistance resources toward making New York the healthiest state.
