The Second Curve of Population Health - Organizational and Infrastructure Transformation

Hospitals and care systems are increasingly moving into the field of population health management to improve the health of their patients and surrounding communities. In this webinar, Maulik Joshi, Dr.PH., President, Health Research and Educational Trust and Senior Vice President of Research, American Hospital Association (AHA); Heather Jorna, Vice President, Health Care Innovation, AHA; and Stephen Martin, Ph.D., Executive Director, Association for Community Health Improvement, AHA will discuss strategies and measures that hospitals and health care systems are focusing on to advance population health management.
In addition, Dr. Martin will discuss the results of a recent AHA/ACHI survey of acute care hospitals on population and community health organizational and staffing infrastructure.
