Requirements for 2014 CSP Submission

HANYS is hosting this webinar in collaboration with the DOH to inform hospitals and health systems about the 2014 CSP requirements. During the webinar, DOH staff will describe the information being requested from hospitals and health systems for their 2014 CSP requirements. This webinar will be recorded for those members who are unable to participate live on the webinar.
Specifically, DOH will discuss how providers should:
- identify the intervention strategies being implemented;
- describe the target population;
- describe the measures being used to track progress in meeting goals, and report baseline and updated data if available;
- identify frequency of partnership activity and key partner organizations; and
- briefly describe the successes and challenges to making progress in this effort.
