Patient Activation Measure™ (PAM) 101

HANYS continues its Delivery System Reform Incentive Payment (DSRIP) education series with this informative webinar on the Patient Activation Measure™ (PAM). This program, presented with faculty from Insignia Health, will help DSRIP teams prepare for implementation of the 11th DSRIP project (2.d.i) by learning the science behind the development of PAM and how to use the results to provide tailored healthcare.

PAM was developed by researchers at the University of Oregon to assess the knowledge, skills, and confidence of patients in managing their own health. The PAM self-assessment tool segments consumers into one of four progressively higher activation levels. Providers can use the results of the tool to better tailor clinical care to patients’ individual needs. This tailored approach can assist patients in better self-management, leading to improved outcomes and lower costs.

HANYS continues to identify tools and resources to assist members in emerging DSRIP PPSs in designing the framework for specific DSRIP projects. We will host a related webinar on November 5 - OMH Presents PSYCKES: What it is and How it can Help Hospitals and DSRIP Performing Provider Systems.
