This three-part series presents a unique opportunity for HANYS members applying for Delivery System Reform Incentive Payment (DSRIP) waiver participation to access valuable insight and advice from a nationally-recognized expert. Hospitals will be able to apply what they learn from these programs to their Performing Provider System’s (PPS) efforts to reduce unnecessary hospital admissions.

What You Will Learn

As suggested by the Agency for Healthcare Research and Quality's Guide to Reducing Medicaid Readmissions and by the DSRIP Implementation Toolkit, numerous services and care teams can be effective to reduce avoidable hospitalizations and readmissions for Medicaid patients. This program will provide specific, action-oriented examples of how to launch and deploy new care teams at different interfaces (emergency departnebt, post-hospital, community) to achieve DSRIP objectives.
