Collaboration and Bundled Payment Sessions Added to HANYS’ Conference Agenda
The agenda for HANYS’ 44th Annual Membership Conference is finalizing quickly, with the addition of two new educational sessions on collaborative relationships between acute and continuing care providers, and on Medicare bundled payment models.
A panel discussion will feature three members who are achieving success with collaboration between acute and continuing care providers:
- Eli Feldman, President and Chief Executive officer, MJHS;
- Jo-Ann Costantino, Chief Executive Officer, The Eddy, and Vice President, Continuing Care, St. Peter’s Health Partners; and
- Jane Shukitis, Vice President, Home and Community Based Services, Unity Health System.
These speakers will discuss how state and federal health care reform are changing relationships between acute and long-term care providers, and why it makes sense for them to work together in partnership. They will provide examples of innovative, successful approaches to collaboration, including initiatives that improve transitions of care (reducing readmissions and difficult discharges) and expand palliative care and other services.
Gloria Kupferman, Vice President, National Information Products, DataGen, will lead a session on using Medicare bundled payment analytics to identify which Diagnosis Related Groups are top targets for bundling and pinpoint the payment variability associated with the inpatient, physician, and post-acute services provided to Medicare beneficiaries during episodes of care. She will also cover different approaches used to analyze and visualize the data, and action steps.
