| New York State Community Health Partnership LOGO Use Guidelines The New York State Community Health Partnership established a community health brand, apple logo to be used to promote community health partnerships and activities throughout the State. If you are interested in using the logo, please review the following guidelines and submit your application to bwalsh@scaany.org and swagner@hanys.org Notification will be provided to you on whether or not you have been approved to use the logo. THE LOGO: The graphic of a blue heart inside a red apple with the words, "Our NY, Our Health" with or without the tag line "Take it on!" is a registered trademark of the New York State Community Health Partnership (NYSCHP). APPLICATION FOR USE: 1. Any organization or entity not a NYSCHP steering committee member that wishes to use the LOGO must seek permission from the NYSCHP. 2. A committee of the NYSCHP will review completed applications using the following criteria:
3. Approved applications will be returned to the applicant for signature of a principal of the organization or project. The NYSCHP reserves the right to accept or reject applications for use of the LOGO.
AGREEMENT TO USE "Our NY, Our Health" LOGO Use of LOGO:1. The LOGO may be used for a program or project that supports health promotion and/or disease prevention, OR in connection with the activities of a partnership or coalition that supports health promotion and/or disease prevention. 2. Applicants can request to use the LOGO for one purpose (for example, on a brochure or a program), OR as part of a public relations campaign for an ongoing community health program, project or partnership. 3. The LOGO may be used in conjunction with another logo used by the applicant, OR it may be used on its own. 4. The LOGO may NOT be used as the sole identity of any institution or business, OR used in conjunction with a public relations campaign of an individual institution or business. 5. The LOGO must only be used for the purpose requested. Licensee must contact the NYSCHP for permission to use the LOGO for any purpose beyond the original agreement. 6. All members of the NYSCHP are licensed and have expressed unlimited approval to use the logo. All members agree to use logo in a manner consistent with goals of Healthy People 2010, CWT for a Healthy New York (NYS DOH/1996) or Touchstones. 7. Licensee may not reproduce the LOGO on any materials (T-shirts, key-chains, etc.) or publications for which money is charged without prior approval of the NYSCHP. The NYSCHP reserves the right to request a fee when the LOGO is used on any article or publication for which money is charged. 8. Licensee agrees to send the NYSCHP copies of all printed material that shows the use of the LOGO or to produce such material when requested by the NYSCHP. 9. Licensee cannot assign rights for use of the LOGO to another entity and has no permission to do so. 10. Licensee has the right to use the LOGO for one year from date agreement is signed, unless otherwise indicated. Licensee must notify the NYSCHP of any plan to use the LOGO beyond one year and receive approval from the NYSCHP. 11. Licensee agrees to notify the NYSCHP promptly of any known unapproved use of the LOGO by others. 12. Licensee agrees that it will not state or imply either directly or indirectly that the Licensee or the Licensee's activities, other than those permitted by this agreement, are supported, endorsed or sponsored by the NYSCHP or its members; and, upon the direction of the NYSCHP, shall issue express disclaimers to that effect. Licensee agrees not to use the LOGO except as authorized by this agreement.
Presentation of LOGO: The LOGO must be used in the format that appears on the website. No variations are acceptable in the presentation of the LOGO except as follows:
