Patient Navigation
Patient navigation was a main focus of the HANYS Breast Cancer Demonstration Project® (BCDP), a six-year project supported by the New York State Assembly and the U.S. Centers for Disease Control and Prevention. In 2002, the HANYS BCDPÒ Advisory Board created a patient navigation toolkit that was expanded and further developed in 2005 as a cooperative effort between HANYS and Pfizer, Inc., in consultation with Dr. Harold Freeman, President of the Ralph Lauren Center for Cancer Care and Prevention in New York City.
Patient Navigation in Cancer Care: Guiding Patients to Quality Outcomes (TM), the second generation of the toolkit, was distributed to HANYS' members and hospitals and cancer care centers across the nation in 2005 to encourage the dissemination of patient navigation concepts and application. Patient Navigation in Cancer Care offers detailed guidance on how to establish and evaluate patient navigation programs and provides resources and training information for patient navigators.
