Resource Kits
To help enhance care for persons with or at risk for breast cancer, the HANYS BCDP Advisory Board authorized the development of several resource kits related to specific outreach and care management strategies. Each kit focuses on a particular program developed by or tested at a HANYS BCDP site and provides a comprehensive template to help you implement that program in your hospital.
Each kit contains materials in a variety of Microsoft formats that you can download and customize for your use. In addition, HANYS can help you customize and print bulk copies of certain sample flyers and brochures. An order form is included where this feature is available.
