Diabetes Campaign Collaborative 2012-2013

The Healthcare Association of New York State's Diabetes Campaign Collaborative, sponsored by the New York State Health Foundation's Statewide Diabetes Campaign, is leading the fight against diabetes in New York by helping providers implement new, coordinated systems of care and prevention.

More than 112 hospitals and 250 clinics participated in HANYS' Diabetes Campaign Collaborative during its first year, resulting in 260 practitioners achieving National Committee for Quality Assurance recognition, with another 800 in the pipeline for year two. The New York State Health Foundation reports nearly 1.8 million New Yorkers have diabetes. Moreover, a recent study conducted jointly by HANYS and the New York State Health Foundation found diabetes to be a key driver of rising health care costs. According to the report, titled The Hidden Health Care Costs of Diabetes, the percentage of inpatient admissions in New York State involving patients with diabetes increased nearly 30% between 2000 and 2008, accounting for 84% of the overall growth in admissions during that period. Furthermore, patients with diabetes are 60% more likely than patients without diabetes to be readmitted to the hospital within 30 days of treatment.

HANYS Diabetes Campaign Collaborative includes a unique set of provisions designed to help hospital based clinics and primary care centers improve outcomes and prevent complications for patients with diabetes, while embracing forward-looking, coordinated practice models that will be most effective in the post health care reform era.

The initiative provides education, technical assistance, and covers some expenses for participating systems' or hospitals' outpatient clinics, primary care sites, and diabetes self-management education programs.
