HANYS to Serve as a New York State Practice Transformation Technical Assistance Contractor
ALBANY, N.Y. — The Healthcare Association of New York State (HANYS) has been awarded a contract by New York State to partner with primary care practices across the state and guide them through the transformation to Advanced Primary Care (APC).
This transformation will ensure that primary care physicians are reimbursed for activities that improve their patients’ overall health. These activities include coordinating patient care among specialists and hospitals, using technology for patients to access their own healthcare, ensuring patients with chronic diseases are managing their conditions effectively, and expanding patient access to methods of communication with their healthcare providers.
“HANYS is honored to serve as a Practice Transformation Technical Assistance Contractor and looks forward to guiding the transformation of many primary care practices across the state,” said Bea Grause, President of HANYS. “Through the APC program, New York State is working to ensure that primary care practices can focus on high-value, preventive services that will improve patient care and save healthcare resources.”
The APC program is directly connected to New York State’s Delivery System Reform Incentive Payment (DSRIP) Program, a larger effort that the state has undertaken to improve care and reduce spending in the Medicaid program. As a part of DSRIP, the APC program will enable Performing Provider Systems to meet critical program milestones, including transforming all primary care practices to a new system of value-based care.
The Healthcare Association of New York State (HANYS) is the only statewide hospital and continuing care association in New York State, representing 500 non-profit and public hospitals, nursing homes, home care agencies, and other healthcare organizations.
