Complex case discharge delay learning series

HANYS is pleased to continue our Complex Case Discharge Delay Learning Series.

During these webinars, experts will provide an overview of strategies to navigate discharge obstacles and help facilitate timely discharge.

This series highlights solutions to help hospitals address the myriad of issues associated with complex case discharge delays. It features themes highlighted in our white paper including guardianship issues, best practices for emergency departments caring for individuals with intellectual disabilities and immigration and legal status.

Upcoming sessions:

HANYS complex case data collection pilot results | Nov. 8

Fifty-two hospitals submitted data to HANYS on patients experiencing discharge delays from April 1 to June 30. During this session, Sarah DuVall, director of behavioral health at HANYS, will provide an overview of the data collection pilot results and an update on advocacy efforts.

Documentation strategies to facilitate timely discharge for individuals with I/DD

Applied behavior analysts with experience in diverse care settings will share strategies to help facilitate timely discharge for individuals with intellectual and/or developmental disabilities.

Insurance eligibility and authorization

Julie Brandfield, JD, Associate Director, LegalHealth, and expert in insurance law, will discuss challenges and share strategies related to insurance eligibility, including considerations for patients with undocumented immigration status.

Partnering with community organizations

During this session, Charles Day, vice president, security, Albany Medical Center, and Joseph Sweeney, senior director, security, Hospital for Special Surgery, shared their strategies for engaging and maintaining relationships with local law enforcement to manage patients experiencing behavior crises.

Family Healthcare Decision Act and Article 17A guardianship

This session offers an overviews of Article 17A guardianship and the Family Healthcare Decision Act provided by Sheila Shea, Esq, director, Mental hygiene legal service, third judicial department, and Robert Swidler, Esq., vice president, legal services, St. Peter’s Health Partners.

URMC project ECHO geriatric mental health

University of Rochester Medical Center discusses integration of behavioral health services into skilled nursing facilities and establishing tele-ECHO clinics that connect URMC specialists with SNF sites across New York.

Caring for individuals with intellectual an developmental disabilities

Leaders from Metro Community Health Centers in Bronx, New York and Center for Disability Services in Albany review levels of support available to individuals with I/DD, discharge obstacles and recommendations for transitions of care, including challenges unique to the COVID-19 pandemic.

Navigating the guardianship process

Expert in guardianship, John Holt, JD, of Project Guardianship, provides an overview of guardianship in New York state and shares insight into navigating the process.
