Antibiotic Stewardship During Care Transitions and HANYS Collaborative Updates

We are hosting this webinar focused on care transitions as part of our Antibiotic Stewardship Collaborative.
Providers across the continuum of care are engaged in efforts to tackle emerging drug resistance. Identifying ways to collect and analyze antibiotic usage data during care transitions has been particularly challenging. Participants in this webinar will learn how one health system has used data to inform its antibiotic stewardship efforts during care transitions, and examples of gaps identified. Acute and post-acute organizations are encouraged to attend. Our goal is to foster provider insight about care transitions across the continuum to advance work in this area.
