Complex Case Discharge Delay Series: URMC Project ECHO Geriatric Mental Health

HANYS is pleased to announce the launch of Complex Case Discharge Delay Learning Series. The webinar series will highlight possible solutions to help hospitals address the myriad issues associated with complex case discharge delays. The series will feature many of the themes identified by attendees at SWAT 2019: Tackling Complex Case Discharge Delays, including sessions focused on guardianship issues and the Family Healthcare Decision Act, best practices for emergency rooms caring for individuals with intellectual disabilities, immigration and legal status and other issues.

The webinar series begins on March 5, from noon to 1 p.m., with Dr. Michael Hasselberg, PhD, RN, NPP-BC, program director of URMC’s Project ECHO Geriatric Mental Health in Long-Term Care. Dr. Hasselberg will present on the work that the ECHO program is accomplishing around increasing the skills, knowledge and self-sufficiency of community-based, long-term care clinicians as they manage older adults with behavioral health and dementia care needs. The program is working to help integrate behavioral health services into skilled nursing facilities and establish TeleECHO Clinics that connect URMC specialists with SNF sites across New York.
