Caring for Individuals with Intellectual and Developmental Disabilities

HANYS is pleased to continue our Complex Case Discharge Delay Learning Series with a session focused on caring for individuals with intellectual and developmental disabilities. Leaders from Metro Community Health Centers in the Bronx and the Center for Disability Services in Albany will review levels of support available to individuals with intellectual and developmental disabilities, discharge obstacles and recommendations for transitions of care, including challenges unique to the COVID-19 pandemic.

HANYS Complex Case Discharge Delay Learning Series highlights possible solutions to help hospitals address the myriad issues associated with complex case discharge delays. The series features themes identified by attendees at SWAT 2019: Tackling Complex Case Discharge Delays, including sessions focused on guardianship issues and the Family Healthcare Decision Act, best practices for emergency departments caring for individuals with intellectual disabilities, immigration and legal status and other issues.

The webinar series launched with a session highlighting University of Rochester Medical Center’s initiative to integrate behavioral health services into skilled nursing facilities and establish Extension for Community Healthcare Outcomes (ECHO®) clinics that connect URMC specialists with SNF sites across New York.
