Patient Centered Medical Home – Overview of core criteria and annual reporting 2022

Practices must demonstrate annually that they have sustained National Committee of Quality Assurance policies and processes to achieve and maintain NCQA Patient-Centered Medical Home recognition. Facing the challenges of the COVID-19 pandemic, many practices understandably had to abandon the workflows, documentation and reporting that were the building blocks of the medical home in order to focus on patient needs.

However, NCQA has made substantial changes to PCMH criteria for 2022 annual reporting, audits can happen at any time and sites are always accountable for any of the core criteria within the PCMH standards and guidelines. Now more than ever, it is imperative to ensure that all core criteria of your medical home are maintained.
