Readiness Assessment: Management of Chronic Diseases
October 12, 2011
2 - 3:30 p.m.
This Webconference is for HANYS’ members only.
HANYS is pleased to announce New Models of Care Delivery: From Strategy to Execution, a new Webconference series that addresses executive level strategic issuesrelated tomaking the business case for new models of care across the continuum, and integrating collaboration into all systems and operations.
In this era of national and state health care reform, providers are challenged to analyze their organizations’ processes for caring for populations with chronic diseases, and to develop and implement new delivery models of care, payment structures, and quality initiatives. This Webconference series will provide information, ideas, and practices that promote “outside the box” thinking for all levels within an organization.
To effectively manage chronic illness, we need to transform primary care delivery using evidence-based approaches and better coordinate chronic illness care between all settings of the care continuum. In this session, participants will learn about the chronic care model (a system transformation strategy) and its close relationship to the patient-centered medical home model. Participants will also review the Assessment of Chronic Illness Care, an improvement readiness survey that helps assess an organization’s alignment with the chronic care model and where improvement can be achieved.
Each Webconference will include a question-and-answer session.
