HANYS, with the generous support from the Milbank Foundation for Rehabilitation, is holding this Webinar for health care providers seeking to initiate or expand access to palliative care for patients, residents, and families. This includes providers interested in developing a palliative care project for Domain 3 of the Delivery System Reform Incentive Payment (DSRIP) program.
While the demand for palliative care services exists across acute and community care settings, the needed resources for the full range of palliative care services―such as palliative care expertise, experience, and knowledge―are not always readily available. Cross-setting collaborative partnerships where palliative care services are deployed across organizational boundaries have resulted in improved access to palliative care for patients, residents, and their families. In this Webinar, leaders from a New York State-certified hospice provider and a health system will discuss their partnership, keys to their success, and best practices to forging new alliances across provider types and care settings.
