Hypertension can do damage to your health before symptoms are even recognizable and put you at risk for heart attack and stroke. One in three Americans have hypertension and only 54% of those patients have their condition under control.
Arnot Health has achieved the highest rates of hypertension control in the nine county Finger Lakes region, according to data released by the High Blood Pressure Collaborative in Rochester. This is due in part to the full transparency of electronic medical records, enabling providers to communicate and exchange patient information and monitor progress. Their primary care practices are Patient-Centered Medical Home (PCMH) Level 3 certified. Case managers contact at-risk patients to offer support, troubleshoot problems, schedule office visits, secure medical transportation, provide medication reminders and assistance, and promote healthy lifestyles.
Over 91.7% of Arnot Health’s registry patients have annual blood pressure readings. Stage 2 patients have blood pressure readings every four to six weeks until their blood pressure is under control. Only 11.5% of uncontrolled patients have dangerously high blood pressure. The control rate varies only slightly, from 83% to 85%, across socioeconomic status, showing Arnot’s success providing care to patients at any income level.
