Chronic conditions can worsen when geography, a lack of insurance, and socioeconomic factors prevent children from getting the ongoing care they need. This is especially true with childhood asthma. As a result, kids with asthma often end up having to go to the emergency room.
Arnot Health at St. Joseph's Hospital in Elmira partnered with the Chemung County School Readiness Project to proactively address the healthcare needs of children with asthma through education and care coordination.
This partnership has decreased the number of children under age four visiting the county's emergency rooms for asthma-related conditions by 33%.
