Eastern Long Island Hospital offers the Senior Options and Solutions (SOS) program to meet the needs of the vulnerable residents in their community who need assistance with independent living. This program provides assistance to older adults, families, and caregivers who need help answering healthcare questions, identifying patient needs, and accessing necessary services.
A care manager contacts all patients who have been identified as high-risk post discharge and is available for in-home assessment to connect patients to beneficial services as needed. In collaboration with community partners, the care managers provide outreach and education on healthcare issues and resources aimed at maintaining health and safety. This promotes local residents' independent living, a sense of self-sufficiency, and prevents deterioration of physical health.
