Little Falls Hospital, in the Bassett Healthcare Network, has been actively engaged in pursuing the Triple Aim for many years. As a Critical Access Hospital, Little Falls Hospital is not currently subject to the requirements of the Centers for Medicare and Medicaid Services (CMS) for Value-Based Purchasing, but recognized the need for transformation within the healthcare industry. With a shared understanding and vision by the board and leadership, support of the Bassett Healthcare Network, and a Vital Access Provider Grant Award in 2014, Little Falls Hospital was able to focus on goals in the following areas: expand access and availability to primary care; reduce hospital readmissions; reduce over-reliance on emergency services of the hospital; and increase the use of observations.
Since 2014, Little Falls Hospital has achieved significant positive performance results in all of these areas, as well as workflow and process improvements. Specifically, Little Falls Hospital has: increased access to primary care by recruiting two new primary care physicians; reduced readmissions to 7.22%; reduced low-acuity visits in the emergency department; increased observation cases; and achieved an overall HCAHPS score of 76%.
