Patient falls present serious safety and quality challenges to patients, families, and healthcare providers. Falls are a leading cause of serious injury to senior citizen populations—and a fall in the hospital adds days on to a hospital stay.
Southampton Hospital initiated a Fall Prevention Program that promotes a culture of safety. Conducting a Failure Mode and Effect Analysis led to process changes in a multitude of areas including documentation, staffing, patient care assignments, and leadership accountability. Enhanced communication, supervisor support, staff education, and an organizational commitment resulted in an initial 29% reduction in the amount of falls in 2008 and from 2008 to 2013 there was an additional 30% reduction.
