St. Luke's Cornwall is Reducing Avoidable Readmissions and Length of Stay

Care coordination is crucial to better healthcare outcomes, especially when a patient is being discharged from the hospital. St. Luke's Cornwall Hospital in Newburgh has developed an innovative program called "Taking it to the Streets: Transforming Care Delivery" that focuses on discharge planning and home follow-up initiatives that help patients and their caregivers play a more active role in their healthcare.

As a result of this program's education and outreach, avoidable readmissions are being reduced and the length of stay for congestive heart failure patients has been reduced from 5.4 days to 4.0 days.
