Health Commissioner Visits Two Hospitals during Public Health Week
To celebrate Public Health Week, April 2 – 6, New York State Health Commissioner Dr. Howard Zucker visited two of HANYS’ member hospitals to highlight some of the important work hospitals are doing in public and community health.
On April 3, Commissioner Zucker hosted Grand Rounds at the Ellenville Regional Hospital with leaders and staff from the Institute of Family Health and the Ellenville Family Health Center. He talked about the future of primary care, and panel participants discussed the importance of collaboration between community health centers and hospitals, integrating social diagnosis into the patient encounter, and the emerging role of telehealth in primary care.
On April 5, Commissioner Zucker visited Montefiore Nyack Hospital to talk about the importance of health literacy in public health and healthcare settings. Montefiore representatives highlighted the work they have been doing to address health literacy and their work with community-based programs. Contact: Sue Ellen Wagner
Innovation Spotlight—Erie County Medical Center Improves Health Literacy
Erie County Medical Center recognized that congestive heart failure (CHF) patients do not always receive sufficient information to manage their condition after leaving the hospital. To confront this issue, a hospital-wide multidisciplinary team developed a health literacy program comprised of multiple educational approaches.
To learn more about this program and its results, read the full story here. This story is just one of the many initiatives profiled on HANYS' NYS Triple Aim website that demonstrate how New York healthcare providers are improving population health, enhancing the quality of patient care, and reducing the cost of healthcare despite fiscal constraints.
