New United Hospital Fund resource assists primary care providers in addressing social needs
To bridge gaps in patient care, the United Hospital Fund has released a framework designed for physicians to not only screen and identify their patients’ social determinants of health but also guide the process of partnering with community-based organizations that can respond to those needs. The framework includes guidance for operational issues and steps before screening, developing partnerships between practice and CBOs, contextual challenges and real-world examples of patients navigating this system.
