Bolstered by community partnership, self-management workshop supports patients with chronic disease

Rochester Regional Health - St. Lawrence Region and The Heart Network are promoting healthy lifestyle changes to address chronic disease. Their self-management program helps reduce hospitalizations and build community for those experiencing chronic conditions.

In 2025, Rochester Regional Health joined Care Connections, a HANYS learning collaborative, to further develop its community partnerships. Care Connections provided data reports on chronic disease prevalence in the hospital’s service area, identified high-priority chronic diseases to address and helped coordinate work with The Heart Network.

Rochester Regional Health - St. Lawrence Region’s self-management workshops are an example of how external partnerships can benefit the community, offering a welcoming space to share experiences, learn practical skills and connect with others who understand chronic disease. Sessions explore ways to reduce stress, stay active, eat well, communicate effectively, set goals, make decisions and solve problems, so patients can feel more in control and live life on their terms.
