HANYS awarded $1 million Mother Cabrini Health Foundation grant for new Care Connections program

HANYS will help hospitals build partnerships to improve community health and address barriers to care

RENSSELAER, NY — With generous support from the Mother Cabrini Health Foundation, HANYS is launching Care Connections to build hospital-community partnerships, address barriers to care, develop a pipeline for community health workers and provide chronic disease prevention and management services.

Care Connections will focus on three regions, which will be announced in early February. Participating hospitals and health systems in the target regions will receive virtual education with expert faculty, evidence-based best practices in chronic disease prevention and management, custom data reports to support decision-making at the local level and technical assistance from a dedicated team of project managers. Hospitals and their local partners may also apply for stipends to support community-level chronic disease interventions and/or community health worker training.

More details on 2025 programming, including recruitment, registration for the upcoming virtual learning series and the stipend programs will be announced soon.
