HANYS awarded two-year renewal from the Mother Cabrini Health Foundation for Care Connections program
Funding will enable HANYS to sustain and grow our chronic disease and community partnership work.
With generous support from the Mother Cabrini Health Foundation, HANYS will extend its Care Connections program through Dec. 31, 2027. This renewal grant will help HANYS facilitate hospital-community partnerships to deliver chronic disease prevention and management services and build a pipeline of community health workers to connect individuals and communities with high-quality, affordable and culturally competent care.
Since 2025, Care Connections has been instrumental in helping hospitals and community-based organizations strengthen their approach to chronic disease prevention and management by:
- assembling a cohort of 28 hospitals across three regions of New York;
- hosting an eight-part virtual learning series on community partnerships and chronic disease;
- awarding $70,000 in stipends to support chronic disease prevention and management projects; and
- awarding three scholarships to support the training of local community health workers.
In 2026, hospitals participating in Care Connections will receive project manager assistance, data analytics and marketing and communication materials. They will also gain tools and resources to identify partners, engage community members impacted by chronic disease and build trust in their communities. More details on 2026 programming, including recruitment, stipend programs and registration for the upcoming virtual learning series will be announced soon.
HANYS is proud to have received awards from the Mother Cabrini Health Foundation since 2020 for hospital COVID-19 emergency relief, improving health equity, addressing workforce shortages and supporting collaboration across the continuum of care.
