Burke Rehabilitation Hospital helps patients get home

Patient access to quality medical rehabilitation services following an acute illness or injury is essential to achieving functional independence and returning to the community. In 2017, Burke Rehabilitation Hospital, a member of the Montefiore Health System, began an initiative to improve the discharge experience at its facility.

Using the Institute for Healthcare Improvement methodology under the Montefiore Performance Improvement Fellowship program, Burke identified trends in length of stay, patient functional improvement and discharge to community that could be improved. To address these areas, Burke created five interdisciplinary care “efficiency teams.” Each team worked closely to analyze patient outcomes and set realistic goals to improve the lives of patients.

The patients have achieved a three-point improvement in their functional level as measured by the Functional Independence Measure. The percentage of patients who are discharged home has increased from 60% to 68%. Additionally, length of stay has been reduced by more than two days.
