SBH Health System Decreases Electronic Health Record Errors

Computerized provider order entry (CPOE) has been shown to decrease errors and is being widely adopted. However, order entry errors also can be caused or exacerbated by a CPOE system. In order to combat patient identification order entry errors, SBH Health System created an innovative Double Patient Identification Alert System. The alert prompts the ordering clinician to actively identify the patient's initials and year of birth prior to placing an order.

The CPOE Double Patient ID System successfully decreased the Emergency Department CPOE Near Miss events by 35% during the initial eight-week pilot period. The system also successfully helped decrease the Emergency Department percentage of all CPOE Near Miss events by 49%. As a result of the success of the CPOE Double Patient ID System, it has recently been implemented in the SBH Health System Intensive Care Unit, Surgical Intensive Care Unit, and Intermediate Medical Care Unit.
